DOCUMENTATION
Prior to appointment patients will receive consents and assessments via e-mail or text. ALL documentation must be completed prior to appointment.

TELEHEALTH
Many appointments can be completed via telehealth, including follow-ups urgent care. Patients will receive links via email and text prior to appointment

CANCELLATIONS
No-Shows and last minute cancelations are subject to a $75 fee. Please notify us within 24 hours to avoid fees. After three last minute cancelations or no-shows, you may be terminated as a patient.
Medications
Refill Requests May Take up to 48 Hours During Regular Business Hours To Process

REFILLS
To expedite your refill requests, we recommend that you either have your pharmacist submit the request electronically or you submit your request through the patient portal.

NO REFILLS REMAINING
If you do not have refills left, this may be an indication that you need to be seen by a provider. Please contact the office one week before you need your medicine.

CONTROLLED SUBSTANCE
Some prescriptions, such as narcotics and amphetamines, cannot be called in unless we have seen you recently or will be seeing you. These will not be refilled after hours. There are NO replacement scripts for lost prescriptions on controlled substances.

Is Dr. Cobb seeing New Patients?
Yes. However, all new patients are seen by a Physician's Assistant first. After the first visit you can schedule with Dr. Cobb.
Are you in Network with Oschner/Blue Cross?
Yes!, we are.
What is your cancellation policy for medical appointments?
Time allocated for an appointment is reserved especially for you. We do understand that sometimes schedule adjustments are necessary; therefore, we respectfully request at least 24 hours notice for adjustments to your appointments and for cancellations. All our policies are designed to benefit our guests and to provide the best quality and tradition of excellent service for our established and future clientele.
Do I have to have a credit card on file?
Yes, we require that all new and current guests supply a credit card to have on our files. All cards on file are added to the system via a secure electronic process that ensures the information is encrypted and remains secure. In the event that we do not receive the required notice for adjustments and cancellations the following fees will be applied to your card or alternatively billed out to you.
What is your No-Show policy?
No-Shows are charged $75. After 3 No-Shows a patient is terminated from the practice.
